•	You must complete the Council driver application form.
•	You must complete a Criminal Records Bureau check form and pay the appropriate fee to them to enable enquiries to be made relating to any offence history.
•	You must produce your DVLA Driving Licence.
•	You must nominate two persons who will support your application and act as referees.
•	You must prove that you meet the required medical standard by arranging for a D4 DVLA medical questionnaire, to be completed by your doctor and submitted to the council with your application.
